Safety and Fire Declaration Forms

The City requires all mobile food vendors to submit declaration forms.  If an item is Not Applicable, the inspector is required to mark off the N/A option and sign off on this as the City does not inspect vendor units.  Incomplete declaration forms will not be accepted. Forms are subject to audit. All relevant declaration forms expire yearly.
