Statement of Damage or Injury Claim Form

If you are claiming for an injury sustained while a passenger of a City of Edmonton Transit Bus you should submit the:

AB1 Notice of Loss and Proof of Claim Form.

Be sure to include your Transit File# and submit within the required 10 business days.  This form should also be available from your Health Care provider and it and all other Accident Benefit Forms are available at the Alberta Finance Website.

For all other claims where you feel the City of Edmonton is responsible for your property damages and/or injury please complete City of Edmonton's:

Statement of Damage or Injury Claim Form.

Send the fully completed and signed form to the Risk Management Section by:

Mail:	to the address noted on the form (if hand-delivered to the 9th Floor Receptionist during regular business hours)

If you do not have access to a computer you may also provide details of your claim on our 24-hour automated voice-recognition system by calling 780-496-5130 and the appropriate form will be mailed for completion.

We will acknowledge your claim by letter, investigate, and respond with either an acceptance or denial.   Please note that depending on the type of claim it may take several weeks (or more) for a determination to be made so we do suggest you also contact your insurer to see if coverage may exist under your insurance policy.
